Interanl Disc Disruption Causing Low Back Pain after Traffic Accident

Abstract
We experienced a patient who complains chronic low back pain not concordant with electrodiagnostic studies and MRI findings after traffic accident. In discography, we found the internal disc disruption which was concordant with low back pain. 42-year-old female had a traffic accident. She was thrown off the bus. She complained pain in low back and bilateral low extremities after the traffic accident. She also complained sitting intolerance. In MRI, there was a disc protrusion in the left posterolateral space of L5-S1 disc level. Conventional therapies had no effect. After 1 year, she was examined through discography for further evaluation. In discography, there was an internal disc disruption at L4-L5 level. We confirmed the lesion using CT immediately after discography. A provocation test was positive by injecting contrast medium. The intradiscal pressure was 15 pounds per square inch (psi) in L4-L5 disc level and 8 psi in L5-S1 disc level. After procedure, pain improved a little.
